E-Statement Only Sign-up Form
_______I elect to opt out of receiving my monthly Bank of Lenox account statement by regular U. S. Mail. Instead, I agree to receive my monthly account statement by E-Statement.
I understand that by signing this form, I will no longer receive my monthly bank statement via U. S. Mail and that I will have 24/7 access to my statement through Internet Banking provided by Bank of Lenox.
I also understand that if at anytime I wish to receive my monthly statement via U. S. Mail, I must notify Bank of Lenox in writing.
There is no fee for using Internet Banking to receive an E-Statement. E-Statements can be printed from any computer by the account holder as often as desired
